Legal Representative form
A legal representative is a person who has legal authority to act on a member’s behalf in making decisions about the member’s health care. Examples of such legal authority are Power of Attorney for health care, court order, guardianship, conservatorship, executor of estate, or health care proxy.
This Blue Cross Blue Shield of Massachusetts (Blue Cross) form is used to document the authority of the legal representative and to authorize Blue Cross to provide the named legal representative with access to the member’s information in accordance with the scope of the legal authority.
