• Your Medicare Health Insurance Card to access your Medicare Part A & Part B effective dates.
• Don't know or have a Part B effective date yet? Just enter an anticipated Part B effective date.
• A list of any medications you’re taking and choice of pharmacy.
• A list of your doctors and health care facilities you visit.
