In-home Care from Landmark Health

We've partnered with Landmark, an independent medical group, to provide our members with access to home-based medical care. Landmark is a leader in medical house calls and team-based, interdisciplinary medical care. Their medical staff includes Massachusetts-based:

  • Clinicians and physicians
  • Dietitians
  • Nurse practitioners
  • Registered nurses
  • Pharmacists
  • Social workers

To participate in this program, you must first be screened for eligibility by a Blue Cross registered nurse care manager. If eligible, this program can grant you access to medical care, support, and educational services in the privacy of your own home. Participation is voluntary, and this service is available at no additional cost to you.

Please note, this program doesn't replace the care you're receiving from your doctor. Instead, your Landmark Health team will work with your current doctors to coordinate your overall care.

Blue Cross Blue Shield of Massachusetts is an HMO and PPO Plan with a Medicare contract. Enrollment in Blue Cross Blue Shield of Massachusetts depends on contract renewal. Anthem Insurance Companies, Inc., Blue Cross and Blue Shield of Massachusetts, Inc., Blue Cross & Blue Shield of Rhode Island, and Blue Cross and Blue Shield of Vermont are the legal entities that have contracted as a joint enterprise with the Centers for Medicare & Medicaid Services (CMS) and are the risk-bearing entities for Blue MedicareRx (PDP) plans. The joint enterprise is a Medicare-approved Part D Sponsor. Enrollment in Blue MedicareRx (PDP) depends on contract renewal.
